Injured Player? How To File An Accident Insurance Claim

Soccer is a physical activity, and unfortunately our players sometimes suffer a serious injury that requires medical attention.  SNSA provides accident insurance for all players who are registered with our Henderson United Youth Soccer (HUYS), Southern Highlands United (SHU), Mini Kickers and High School Spring Training leagues through our Soccer Association for Youth national affiliate.  The accident insurance is primary for players that do not have medical insurance coverage, and secondary for players that have primary medical insurance coverage.  If your child suffers an injury at a soccer practice or game, follow these steps to file a claim: 

  1. The child's parent sends an email to info@snsasoccer.com to notify SNSA about the injury and to start the insurance claim process.
  2. SNSA will request the initial Incident Report Form (part one) from SAY Soccer, and will forward it to the parent of the injured player.
  3. SNSA reviews the completed Incident Report Form received from the injured player’s parent and if approved, forwards the form to the SAY Soccer National Office.
  4. The SAY National Office reviews the Incident Report Form, and if approved, mails the second participant incident form (part two) to the parent along with instructions.
  5. The parents of the injured player will work with K&K insurance directly from this point forward.
